For Zyrtec, see this dosage guide.

Important Instructions

  • Always read and follow the label instructions.
  • Whenever possible, use the dosing device that came with the product. Syringes and droppers are more accurate than teaspoons.

CHADIS

CHADIS is a web-based screening, diagnostic and management system that administers and analyzes pre-visit, online questionnaires completed by parents, teens and teachers, and provides us with instant access to valuable clinical data and resources.

CHADIS questionnaires are an integral part of well child and behavioral health visits and help us to provide the highest quality of care. They are available online and need to be completed prior to your child’s scheduled appointment. For children age 12 years and up, both a parent and child questionnaire need to be completed.

Most insurance companies cover CHADIS questionnaires; however, it is your responsibility to verify coverage with your insurance company. If any questionnaires are denied by insurance, the cost will be your responsibility. For more information, please see the CHADIS FAQ.
