All About CHADIS

We have introduced a helpful tool called CHADIS, which is a secure, HIPPA-compliant service where our doctors order questionnaires to be completed by you or your child prior to their Well Visit. The answers reported through the CHADIS questionnaires are confidential and sent directly to your doctor. Click the links below to learn about CHADIS:

Closed for lunch: 12 pm – 12:45 pm.

Saturday
8:15 am - 12:00 pm

We require an appointment for your child to be seen.

After-Hours Care

If your child’s illness or injury is life threatening, dial 911.

For urgent matters that cannot wait until business hours, call our office to be connected with our answering service. They can connect you with a registered pediatric nurse or the doctor on call, if needed.

For all other routine calls such as medication refills, dosing questions, etc. please call during our regularly scheduled hours.
